Recurrence of malignant pleural mesothelioma at the resection borders--local or distant recurrence?

چکیده
Journal of Thoracic Oncology® • Volume 8, Number 7, July 2013 We congratulate Gomez et al for their very important contribution on the patterns of failure, toxicity, and survival after extrapleural pneumonectomy and hemithoracic intensity-modulated radiation therapy, for malignant pleural mesothelioma. Studies about patterns of failure are of great importance because patterns of local and distant failure might be the most important test of adequate treatment planning and treatment success. After extrapleural pneumonectomy and hemithoracic intensity-modulated radiation therapy, only 16% of the patients experienced local recurrence, whereas, distant recurrence was observed in 59% of the patients. If we go into details, the predominant site of distant recurrence was contralateral hemithorax in the majority of the patients (41%; n = 35) followed by abdomen and pelvis, including liver in 28% of the patients (n = 24). In general, distant recurrence is defined as tumor that has spread to organs or tissues distant from the primary tumor. However, contralateral hemithorax and abdomen are located directly at the resection borders. Contralateral pleura and peritoneum could be opened accidentally during extrapleural dissection in the anterior or posterior mediastinum and diaphragmal resection. Local recurrence model of malignant pleural mesothelioma for investigation of intrapleural treatment.
OBJECTIVE Local recurrence remains a major problem in the treatment of malignant pleural mesothelioma. The aim of the underlying study was to establish a standardised local recurrence model in rats which enables to study different intrapleural therapies. MATERIALS AND METHODS Fifty microlitre containing 1 x 10(6) cells of a syngeneic rat malignant mesothelioma cell line (II-45), established f...
متن کاملSurvival and relapse pattern after trimodality therapy for malignant pleural mesothelioma.
PURPOSE Multimodality therapy has been applied to resectable malignant pleural mesothelioma, but the tolerability of the treatment and relapse pattern in detail remain unknown. We reviewed our experience of trimodality therapy as a single-institution study in Japan. METHODS A total of 16 patients with resectable malignant pleural mesothelioma were intended to treat with extra-pleural pneumone...
متن کاملClinical outcome of postoperative highly conformal versus 3D conformal radiotherapy in patients with malignant pleural mesothelioma
BACKGROUND Radiotherapy (RT) is currently under investigation as part of a trimodality treatment of malignant pleural mesothelioma (MPM). The introduction of highly conformal radiotherapy (HCRT) technique improved dose delivery and target coverage in comparison to 3-dimensional conformal radiotherapy (3DCRT). The following study was undertaken to investigate the clinical outcome of both radiati...
متن کاملSurgical treatment of malignant pleural mesothelioma: a review.
Despite many years of clinical research, there is still no effective therapy for malignant pleural mesothelioma (MPM). Untreated, the prognosis is poor, with a median survival of < 1 year. Single-agent or combination chemotherapy as well as radiotherapy have not shown persistent improvements in response or survival. In general, MPM is a disease confined to the pleural cavity for a long time bef...

Background: Colorectal cancer is the most common malignant gastrointestinal cancer and the fourth most common cancer throughout the world. Disease staging is performed to select the treatment method. In this interventional study we evaluated the effect of adjuvant chemotherapy on local recurrence, distant metastasis and three-year disease free survival in patients with stage 2 colorectal cancer...
